Reorg troubleshooting (#150)

* Reorganize troubleshooting section

- Classified in subcategories hopefully more logically

* Reorganise troubleshooting section step 2

Now put crust debugging later. I am not sure this is fundamental, but
maybe just showing my lack of experience here...

* Improve table of contents

- Use short ToC entry when needed
- Remove extra space now that we do not have large numbers in
  section names anymore
